Our Health Equity grants advance the following outcomes:

  1. Well-connected stakeholders working together to implement shared goals
  2. An effective, inclusive, and sustainable leadership infrastructure for health equity exists in New Hampshire
  3. Communities of color are more empowered to self-advocate and participate in leadership
  4. White leaders are more prepared to serve as skilled allies in addressing systemic racism and promoting equity
  5. Health equity is more widely understood
  6. Stakeholders use knowlege base to take actions that promote health equity
  7. Enhanced capacity to collect, analyze and use cross sector data
  8. Communities are effectively implementing strategies to create more Welcoming Communities
  9. Policies and programs are formulated and considered through a health equity lens
